What is Chronic Care Management (CCM)?

According to the Centers for Medicare and Medicaid Services (CMS), approximately 93% of Medicare spending is on individuals with multiple chronic conditions. As the population ages, the number of such patients is increasing.

Studies show that 41% of patients with multiple chronic conditions do not take medications as prescribed, with a resultant cost of over $100 billion annually spent on patients who are non- compliant with their treatment plans. This is mostly a result of lack of full understanding of their treatment plan, their conditions, and their personal responsibility.

With current reimbursement rates physicians are forced to limit the time they can spend with each patient during their office visit. This becomes a barrier to patient's full understanding of their plan of treatment. CCM program allows doctors to reach out to patients when they are not in office to reinforce and assure their adherence to treatment plan.
